Online registration is *NOW OPEN* to become a C.A.S.U.A.L. Day Team Captain!  Click HERE to fill out the ‘Team Captain Registration” form.  Within a few days,  the Cancer Institute will send you a username to the e-mail address you provide.  You can then begin ordering your 2014 C.A.S.U.A.L.
